We have converted from paper and have been "live" for all of one week.  Our previous handwritten discharge instructions needed to be reviewed and co-signed by two nurses, before being reviewed with the patient and family.   I am interested if any other facility requires two signatures with their Meditech discharge process.

We only have one nurse sign the paperwork after reviewing with the patient.

The nurse who explains the instructions to the patient signs our discharge instructions, as well as the patient, and the physician.

Judy et al, I am curious about the system you use in MT for patient discharge. Is it the MT routine or is there a third party vendor that you use for the D/C process?  Guess the other question is what version of MT are you using?  We are on C/S 5.65.

We do not co-sign the D/C documentation and never did on paper either.

We are 6.06, and do not use a third party vendor.

We are 6.05, migrating to 6.07 and we do not use a 3rd party vendor for the actual discharge. We do use Ebsco for patient education materials printed with the MT discharge.

We are 5.65 and are using the Meditech discharge instructions

We use 6.06 and can print out our discharge instructions and discharge education sheets from the discharge menu button.  We are using it minimally at this time but will be expanding this process in the future.

So how do you set this up in Meditech C/S 5.65 or 5.66?

For those of you who do not use a third party vendor, how do you provide discharge instructions? We are going to 6.07. We are currently using Exit Care for discharge instructions/meds but we are implementing Dr.First for discharge meds in early 2014. Apparently Dr. First does not provide discharge information. Therefore, we are contemplating what we need to do for discharge information/instructions.

We have a few discharge instruction pdocs built, based on paper forms our physicians were using.  One is for medical patients, another for surgery patients, and we've built one unique set for total knee discharges.  The reason for the total knee instructions is to allow surgeons to have separate norms since these patients have very uniform instructions.  It's probably something we'll end up doing for other specialties as well.
